Probably the Minimoog/SEM combo he was using at the time. IIRC, the Oberheim had a real nasty sounding PWM. I think JH was also using either Maestro or Tycobrahe phasers and flangers.

If possible, replace the whole jack. They're not that expensive, and chances are, you'll probably bung up one of the contacts trying to fix it anyway.
Buy 2 replacement jacks, preferably not from Radio Shack. That'll save you a possible extra trip. :)
